Foundayo vs. Wegovy Pill: What Each One Really Costs Per Month (2026)
There are now two FDA-approved GLP-1 pills for weight loss: Novo Nordisk’s Wegovy pill (approved December 22, 2025) and Eli Lilly’s Foundayo (approved April 1, 2026).
Both are advertised at $149 a month. That’s the price of the starting dose. Neither pill stays at $149, and the two price ladders climb differently.
We priced every dose from both manufacturers’ own pages, read the refill rules, and worked out what a first year of self-pay treatment could cost.

Quick Answer: Which GLP-1 Pill Costs Less?
At the manufacturer’s self-pay prices, Foundayo usually costs a little less in the first year, mostly because its early doses are cheaper. Once both reach higher doses, the monthly price is the same: $299.
That answer changes if you have commercial insurance (both makers offer savings cards), if you’re on Medicare, if you get the pill through a telehealth company that charges a membership fee, or if you miss Foundayo’s 45-day refill window.

What Could Your First Year Cost?
Here’s what 12 months of manufacturer self-pay could look like, assuming your dose goes up every 30 days until you reach the dose shown and then stays there.

Don’t forget the prescriber
Both pills require a prescription. If you get it through a telehealth company, you may pay a membership fee on top of the medication.

Truth or Hype? “The Two GLP-1 Pills Cost the Same”
Mostly hype.
Both start at $149 and both top out at $299. But between those two numbers, Foundayo’s cheaper early doses usually make its first year cost less. Its 45-day refill rule can cancel that out if you refill late.
“Same price” is only true at the two ends of the price ladder. Most people spend their first few months in the middle.
Beyond Price: Other Differences That Matter
How you take it
This is the most practical day-to-day difference. The Wegovy pill has to be taken first thing in the morning on an empty stomach, with no more than 4 ounces of water, and you have to wait at least 30 minutes before eating, drinking or taking other oral medications. Foundayo can be taken at any time, with or without food.
If your mornings are unpredictable, or you take other medications first thing, that difference may matter more to you than a few dollars.
Weight-loss results (read carefully)
In its main trial, the Wegovy pill produced about 14% average weight loss at 64 weeks, counting everyone whether or not they stayed on treatment (about 17% for people who stayed on it). Foundayo’s highest dose produced 12.4% average weight loss at 72 weeks in its main trial.
These were different trials with different people, durations and methods. They weren’t a head-to-head comparison, so these numbers alone don’t show that one pill works better than the other. Individual results vary.
Safety
Both carry a boxed warning about possible thyroid tumors, including cancer. Common side effects for both include nausea, diarrhea, constipation and vomiting. Discuss your own history with a licensed prescriber, and read each drug’s full prescribing information.
Are You on Medicare? This Could Change Everything
Starting July 1, 2026, the Medicare GLP-1 Bridge lets eligible Medicare Part D enrollees get Foundayo, Wegovy (pill or injection) or Zepbound KwikPen for $50 a month. The program runs through December 31, 2027.
Key eligibility points from Medicare’s fact sheet:
- You need Medicare Part D coverage (a standalone drug plan or a Medicare health plan).
- You must be 18 or older and meet BMI and health criteria, for example a BMI of 35 or higher, or lower BMIs with certain conditions.
- You’re not eligible through this program if you have type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease.
- Your doctor has to submit a form to Medicare for approval.
- The $50 does not count toward your Part D deductible or yearly out-of-pocket limit.
Have Commercial Insurance?
Lilly says eligible people with commercial insurance may pay as little as $25 a month for Foundayo with its savings card.
Novo Nordisk offers savings programs for the Wegovy pill as well. Terms and eligibility apply, and your plan’s coverage decides what you actually pay.

What We’d Check Before Paying for Either Pill
- What will I pay at the dose I’m likely to end up on, not just the starting dose?
- Is this the manufacturer’s price, or a telehealth company’s price with a membership on top?
- Is there a refill deadline (like Foundayo’s 45 days) that changes the price?
- Does the price I’m quoting expire? Several current prices are listed through December 31, 2026.
- Can I realistically take the Wegovy pill on an empty stomach every morning?
- Do I qualify for a savings card or the Medicare GLP-1 Bridge?
- Who adjusts my dose and handles side effects, and is that included?
Bottom Line
The GLP-1 pills are the same price at the start and at the top, but not in between.
Foundayo’s cheaper early doses and flexible timing are real advantages. Watch the 45-day refill rule at the higher doses.
The Wegovy pill has a strong trial result and uses the same drug as the Wegovy injection, but its morning routine is stricter.
Whichever you’re considering, compare what you’ll pay at your likely dose, including any membership, not the $149 headline.
